Novel Use of the “Contra Catheter” (CC) In Proximal Iliac Artery Chronic Total Occlusion (PICTO) Recanalization

چکیده

Proximal Iliac Artery chronic total occlusions (PICTOs) are complex peripheral interventions that typically require an arm approach for adequate leverage. These are typically TransAtlantic Inter-Society Consensus [1] (TASC) Type C/ D lesions that have traditionally treated by Aorto-bifemoral surgery [2] but are being increasingly approached by endovascular methods [3,4]. The endovascular approach [4] may be the only option in some patients who are poor surgical candidates due to age, lung disease and comorbidities that typically seen in patients with severe PAD. In addition, endovascular approaches have inherent advantages in that they are less invasive with a shorter length of stay and early recovery and can be performed with local anesthesia [1] and can provide durable results [5].

Balloon Occlusion of the Contralateral Iliac Artery to Assist Recanalization of the Ipsilateral Iliac Artery in Total Aortoiliac Occlusion: A Technical Note

Endovascular recanalization of chronic total aortoiliac occlusion is technically challenging. Inability to reenter the true aortic lumen, following retrograde iliac recanalization, is one of the most common causes of failure. We describe a case of a total aortoiliac occlusion where balloon occlusion of the right common iliac artery, following its recanalization from a brachial approach, was use...

Recanalization of a Coronary Chronic Total Occlusion by a Retrograde Approach Using Ipsilateral Double Guiding Catheters

The retrograde approach through a collateral artery is now thought to improve the success rate of percutaneous coronary intervention (PCI) for coronary chronic total occlusion (CTO), and different kinds of strategies for this technique have been developed. However, the basic principles of PCI for CTO, such as firm back-up support with a guiding catheter and fine control of the guide wire, shoul...

Technical tips and procedural steps in endovascular aortic aneurysm repair with concomitant recanalization of iliac artery occlusions

PURPOSE The goal of this paper is to describe our technical strategy and procedural steps for endovascular aneurysm repair (EVAR) when performed with concomitant recanalization of the iliac artery occlusion. MATERIALS AND METHODS Three octogenarians having abdominal aortic aneurysm (AAA)/common iliac artery aneurysms (CIAA) with unilateral external iliac artery (EIA) occlusion underwent EVAR ...

Angioplasty of chronic total coronary occlusions with the use of six French guiding catheters.

The efficacy of 6 French guiding catheters for angioplasty of chronic total coronary occlusions was evaluated in 61 consecutive patients. The duration of the occlusion was determined angiographically, or estimated from an index clinical event. Endpoints were procedural success, defined as recanalization with less than 50% residual stenosis without major complications; and the need to change to ...

Chronic total occlusion in ostium of right coronary artery – retrograde approach as the first-choice method of revascularization?

Recanalization of chronic total occlusion (CTO) located in the ostium may require the operator's ability to use the retrograde approach. We present a case of opening a chronically occluded right coronary artery (RCA) by the retrograde approach after an unsuccessful attempt of recanalization by classic antegrade technique.
